What is the purpose of this form?
This form is used by car park owners, operators or authorised representatives, who want to notify us of changes to their details and are already registered with the State Revenue Office.
Who can complete this form?
This form must be completed by the owner/operator of the car park or authorised representative as applicable.
How is the form to be lodged with SRO?
This form is completed and lodged online. When you have completed all relevant and mandatory information, please select submit. The information provided will then be forwarded electronically to the SRO and you will be directed to a receipt page. Please download and save a copy of the receipt from this page.
Please note: This form must be lodged with the SRO within 1 month of the change occurring. A penalty may apply for failing to comply with the requirement to notify the SRO of certain changes. See SRO website for current penalty rates.

Explanatory notes
Change details
1. New owner/new authorised representative for owner
This part should only be completed by a registered owner. Please only enter the details that have changed. To nominate a new authorised representative to act on your behalf, provide full details for that person.
Upon receiving this form, we will deal directly with your nominated representative. All correspondence and information relating to all the car parks on your premises will be sent to your authorised representative, except for public car parks where an operator has been nominated.
2. New operator/new authorised representative for operator
This part should be completed by a registered operator if their details have changed, or if they have started or stopped operating a public car park.
3. New car park
Provide details of premises purchased or newly created car park(s) you own.
Separate entries are required for parking spaces at different locations, which will be treated as separate car park(s). If more than one car park exists at one address, please complete the information requested for each car park.
Example
You own a multi-story building, which has 5 floors of parking spaces; 3 of the floors are operated as a public car park by a tenant/operator and the other 2 floors of parking spaces are managed by a building manager as a private car park. You must enter separate details for each car park by repeating the street address and identifying the floors that operate as a private car park and those that operate as public car park.Address of premises in which the car park is located
For each parking space that you own, please provide details of the location, such as building name and street address, including floors or levels of the building.
Car park type
Please indicate whether the car park is a public car park open to members of the general public or a private car park.
Spaces
Provide the total number of marked spaces owned by you in the car park, including parking spaces that may be exempt. There will be an opportunity to claim exemptions for spaces in the Annual Return for your car park(s).
Area used for parking (square metres)
If all, or part, of a car park comprises areas without marked parking spaces, please indicate the extent — in square metres — of these areas used for parking. For information on what constitutes a parking space, please refer to car park information.
Name of operator
For a public car park operated by a person who is not the owner of the premises, please provide the name of the person operating the car park.
Date of possession
The date on which you (the owner) took possession of the premises.
4. Premises sold/car park stops operating
Provide details of all parking spaces existing on premises owned or operated by you that have either been sold or stopped being used as a car park. Separate entries are required for parking spaces at different locations, as these are treated as separate car park(s). If more than one car park exists at the one address, complete the information requested for each car park.
Building ID
Provide the building ID, if known, from your last Annual Return.
Address of premises containing the car park
For each car park where you own or operate some or all of the parking spaces, provide complete details of the location, such as building name and street address, including floors or levels of the building.
Purchaser’s name
Please provide name of new owner(s).
Date of possession or cessation
The date when the purchaser took possession of the premises or date when the premises stopped being used as a car park.
Additional information
This form must be completed by:
The registered owner of a car park if:
- There is any change to their contact details.
- They acquire new premises from which a car park operates.
- They cease to be the owner of the premises.
- The premises cease to operate as a car park.
- They own premises from which a new car park starts operating.
- They wish to change their authorised representative.
The registered operator of a car park if:
- There is any change to their contact details.
- They cease to operate a public car park.
- They commence operating a public car park.
This Change of Details form must be lodged with us within one month of the change occurring. A penalty may apply for failing to comply with the requirement to notify us of certain changes.
If you are an owner or operator who is not currently registered with us, do not complete this form. Please complete the application form for registration as an owner or operator of a car park.
