You do not need to complete an application form, but make sure you clearly describe the documents you are requesting and include your contact details.

If you are making a request on behalf of another party, please include a signed authority from them to show us you have their permission to obtain the requested information.

When making a request, include proof of payment for the statutory application fee or evidence in support of waiving the fee, such as a valid Pensioner Concession Card or Health Care Card.

From 1 July 2021 to 30 June 2022, the statutory application fee for lodging an FOI request is $30.10. Payment can be made by electronic funds transfer (EFT). Please contact our FOI team for payment details.

We aim to provide an access decision within 30 days of receiving a valid request, although some decisions may take longer. We will notify you if we are unable to provide a decision within 30 days.
