What is the purpose of this form?
This form, developed as part of the payroll tax harmonisation initiative, can be used by employers seeking an exclusion from grouping. For more information, please refer to Revenue Ruling PTA-031v2.
Who can complete this form?
This form should be filled in by a representative of the employer applying for an exclusion.
If your businesses are related corporations under the Corporations Act 2001, you are precluded from applying for an exclusion from grouping
